Patient Zero earns its keep by taking a familiar outbreak setup and giving it a genuinely intriguing twist: what if the “monsters” could talk back, negotiate, even outthink you? Stefan Ruzowitzky stages the film with a lean, underground-bunker claustrophobia that keeps the tension simmering, while Matt Smith brings nervous, human urgency to a role that lives at the uneasy border between empathy and survival; Natalie Dormer adds steel and vulnerability that keeps the story grounded. The vibe is gritty and high-strung—part action-thriller, part moral pressure cooker—more interested in uneasy questions than cheap jump scares, even when the violence hits hard. It won’t convert viewers who want their zombie-adjacent horror purely relentless or purely cerebral, but if you’re on the fence and want a watchable, intermittently intense ride with a strong cast and a hook you don’t see every day, it’s an easy “press play.”

Patient Zero wants to be a thoughtful twist on the zombie-pandemic formula, mixing action with a moral question about whether the “infected” are monsters or the next step in evolution. The problem is it plays like a patchwork of better outbreak movies: the plot beats are easy to call, the tension rarely lands, and the middle stretches feel padded while the villains and conflicts stay oddly thin. Matt Smith brings real intensity and Natalie Dormer adds a bit of heart, and there are a few solid bunker-set moments, but the film never fully cashes in on its promising “translator to the infected” hook.
