Guy Luthan, a British doctor working at a hospital in New York, starts making unwelcome enquiries when the body of a man who died in his emergency room disappears. After the trail leads Luthan to the door of an eminent surgeon at the hospital, Luthan soon finds himself in extreme danger from people who want the hospital's secret to remain undiscovered.

Extreme Measures is the kind of smart, adult thriller they don’t make often anymore: brisk, unsettling, and powered by an idea that keeps tightening in your mind long after the credits. Michael Apted turns a hospital into a moral pressure cooker, where one missing body becomes a corridor-by-corridor descent into questions about who gets protected, who gets used, and what “progress” is allowed to cost. Hugh Grant gives one of his best dramatic performances as a doctor whose decency becomes a liability, while Gene Hackman brings that formidable calm that makes every conversation feel like a warning. It’s not for the squeamish—there’s graphic ER imagery and a harsh edge—but if you like thrillers that are as literate as they are tense, this one is a quietly gripping ride.
Extreme Measures wants to be a smart, morally thorny medical thriller about how far “progress” can go before it turns into cruelty, and on paper that’s a solid hook. The problem is the movie keeps piling on twists and conspiratorial setbacks that start to feel more preposterous than tense, so the ethical debate gets buried under melodrama and convenience plotting. Hugh Grant is an engaging, anxious everyman and Gene Hackman brings gravitas, but the pacing can lurch and the social commentary never fully clicks into something satisfying. Also, be warned it’s not squeamish-friendly: the ER gore and intensity are pretty graphic, so if you’re here for a grounded, believable thriller, this may waste your time.
